Job summary

ST. CHARLES HEALTH SYSTEM is seeking an Ambulatory Care Clinical Pharmacist in Bend, OR to provide specialized pharmacotherapy within a collaborative care team. You will develop pharmacotherapeutic goals, select regimens, and create monitoring plans tailored to individual patients.

You will lead efforts to prevent medication errors, improve safety and efficacy, and support ongoing professional development of pharmacists, technicians, and students within the ambulatory setting.

Qualifications

• Graduate of an ACPE-accredited College of Pharmacy.
• Doctor of Pharmacy or other advanced degree preferred.

Responsibilities

• Collaborates with health care providers to create and maintain CDTM agreements to improve medication selection, monitoring, and adherence.
• Prescribes and manages specialty and other medications upon referral to ensure cost-effective therapy.
• Performs comprehensive medication and adherence management and documents in the patient record.
• Coordinates removal of barriers to needed therapy including prior authorizations and financial assistance.
• Educates patients and families on goals, schedules and potential side effects of treatment.
• Supervises pharmacy technicians and mentors students and residents as needed.
• Provides liaison with medical and nursing staff and ensures compliance with regulations and standards.
