St. Charles Health System Inc. seeks an Ambulatory Care Clinical Pharmacist to optimize medication therapy within a specialized healthcare team. The role involves creating pharmacotherapeutic goals, selecting regimens, and monitoring plans tailored to patient needs.

The pharmacist leads safety initiatives, reports medication errors, and collaborates with LIP and the multidisciplinary team to improve care quality. ACPE pharmacy degree is required; Oregon licensure is mandatory.

Formación

• Graduate of an ACPE accredited College of Pharmacy.
• ASHP PGY-1 residency or 5 years of relevant clinical pharmacy experience.
• Current license or eligibility to practice as Pharmacist in Oregon.

Responsabilidades

• Collaborate with providers to manage drug therapy and monitor outcomes.
• Prescribe and manage medications through CDTM agreements as applicable.
• Educate patients and families on treatment plans, risks, and adherence.
