Job summary

St. Charles Health System in Bend, Oregon is seeking an Ambulatory Care Clinical Pharmacist to provide expertise within a collaborative healthcare team and optimize patient drug therapy.

You will develop pharmacotherapeutic goals, select regimens, monitor outcomes, and lead efforts to improve safety and cost-effectiveness. The role emphasizes CDTM, patient education, coordination with LIPs, and mentoring of pharmacy staff.

Qualifications

• Graduate of an ACPE-accredited College of Pharmacy.
• ASHP PGY-1 residency or 5 years of clinical pharmacy experience.
• Current Oregon Pharmacist license or eligibility.
• BCACP/BCPS or related certification preferred.
• AHA BLS certification preferred.

Responsibilities

• Collaborates to create, maintain and use CDTM agreements to improve drug therapy and adherence.
• Prescribes and manages specialty and other meds upon referral from LIPs.
• Performs comprehensive medication and adherence management and documents in medical records.
• Coordinates barriers to therapy including prior authorizations and financial assistance.
• Educates patients and families on goals, schedules and possible side effects.
• Mentors pharmacists/technicians and supports students and residents.
