Job summary

St. Charles Health System in Bend, Oregon seeks an Ambulatory Care Clinical Pharmacist to optimize pharmacotherapy within a collaborative care team. You will develop pharmacotherapeutic goals, select regimens, and monitor therapy to meet patient needs.

You will lead efforts to prevent medication errors and adverse events, develop CDTM agreements, and collaborate with LIP, providers, and pharmacy staff to improve safety and efficacy of treatment.

Qualifications

• Graduate of ACPE-accredited College of Pharmacy.
• Oregon pharmacist licensure or eligibility.
• ASHP PGY-1 residency or 5 years of clinical experience.
• PGY-2 specialty or ambulatory experience preferred.
• Board certifications in specialty areas preferred.

Responsibilities

• Collaborates to create CDTM agreements for improved therapy decisions.
• Prescribes and manages specialty medications upon LIP referral.
• Performs medication and adherence management; documents in EHR.
• Addresses prior authorizations, benefits investigations, renewals, and financial aid.
• Monitors drug interactions, toxicities and costs; follows guidelines.
• Educates patients/families on treatment goals, timing and side effects.
• Mentors pharmacists/techs and precepts students/residents.

ACPE-accredited College of Pharmacy

Doctor of Pharmacy or advanced degree (preferred)
