Job summary

Piedmont Heart Institute Physicians is seeking a Nurse Navigator to coordinate the evaluation and screening of patients being evaluated for complex cardiac surgery. You will serve as the primary point person for the valve team, including physicians, patients, and other clinicians.

The role involves coordinating inter-departmental processes, making recommendations to the physician team, and guiding patients through the care pathway.

Qualifications

• Graduate of an accredited nursing program and licensed to practice.
• Minimum 3 years of nursing experience in cardiology or critical care.
• Experience with protocol development, quality improvement and regulatory readiness.

Responsibilities

• Coordinate evaluation and screening for cardiothoracic surgery patients.
• Serve as the primary point person for the valve team including physicians and patients.
• Coordinate inter-departmental processes and communication.
• Make recommendations to physician team for final determination.
• Provide patient navigation throughout the care pathway.
