Responsibilities

• Coordinate care across the continuum and act as primary contact.
• Review medications, manage refills, and ensure EMR accuracy.
• Provide patient/family education and develop a cross-visit plan of care.
• Support pre-visit planning and post-visit wrap-up with care team.
