Job summary

Mercy Health seeks an experienced RN Navigator for Cardiac Physicians at Marshall Nemer Pavilion in Kentucky. The RN Navigator guides patients through the health system, coordinating care, scheduling, and connecting them with resources to ensure a smooth care journey.

It involves bedside collaboration across multi-disciplinary teams and ongoing patient education. The role requires an RN with at least 3 years of direct patient care, state licensure, and preferred certifications in oncology or

Qualifications

• 3 years of direct patient care experience as an RN (required)
• Licensed Registered Nurse in state of practice (required)
• BLS Basic Life Support - preferred upon hire, must obtain prior to direct patient care
• Oncology Nurse Navigator - ONN-CG certification (preferred, if placed in Oncology)
• Breast Health Clinical Navigator - BHCN certification (preferred, if placed in Mammography)

Responsibilities

• Guides patients in understanding diagnoses, treatment options, and care process.
• Serves as the essential link for communication between patients and all care providers.
• Coordinates and advocates for care across multidisciplinary teams and documents recommendations.
• Monitors patient progress and communicates with the care team to adjust treatment plans.
• Facilitates appointments for consultations and support services within service standards.
• Participates in patient education programs, community outreach, and quality improvement initiatives.
• Tracks patient outcomes and prepares reports to support quality initiatives.
