Job summary

MercyOne Iowa Heart Center Des Moines is hiring an RN Navigator: Heart Failure/Cardiac to support a Center of Excellence for Heart Failure Management within Trinity Health. The navigator coordinates care across the continuum, provides patient and family education, advocates for patients, and collaborates with clinicians to ensure seamless transitions from hospital to home.

This full-time day shift position reports to the Director of Cardiac Specialty Services and requires Iowa RN licensure, BLS,

Qualifications

• Registered nurse with valid license in State of Iowa.
• Bachelor’s degree in healthcare field or actively pursuing, BSN preferred.
• Two (2) years of related work experience required, - 4 years preferred.
• Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire.
• Excellent communication skills and presentation skills.
• Valid BLS certification required within 3 months of hire.

Responsibilities

• Coordinated care for all appropriate patients throughout the continuum of care from diagnosis through rehabilitation.
• Reconciles daily census for identifying heart failure patients in hospital and conducts daily rounds.
• Provides patient/family education and coordinates discharge planning and post-discharge follow-up.
