Job summary

MercyOne Health System in Des Moines, IA is seeking an RN Navigator for the Heart Failure/Cardiac Surgery team. The role coordinates patient care, assists surgeons, and educates patients and families about care plans.

It supports scheduling and ensures efficient care across health care areas. The ideal candidate holds an Iowa RN license, a BSN is preferred, and has 4 years in the specialty with BLS and Mandatory Reporter training.

Qualifications

• Current Iowa registered nurse (RN) license.
• Bachelor’s degree in health-related field or actively pursuing; BSN preferred.
• Demonstrates competency with 4 years in the specialty.
• Proof of Mandatory Reporter training within 3 months of hire.
• Basic Life Support certification within 6 weeks of hire.
• Demonstrates leadership experience.

Responsibilities

• Coordinate with cardiothoracic team scheduler for timely care.
• Educate patients and families on care plans per surgeons.
• Follow up on test results and plans with care teams.

Join the MercyOne Family! We are looking to hire an RN- Navigator: Heart Failure for our MercyOne Iowa Heart Center Des Moines.

As a RN Navigator Cardiac Surgery at MercyOne, you will coordinate and provide care for cardiothoracic surgery patients within MercyOne Iowa Heart Center in Des Moines, IA. Assists cardiac surgeons and advanced practice providers with coordinating patient care including pre-operative testing, working with the MercyOne Des Moines OR on surgery scheduling, and overall optimizing care for cardiothoracic surgery patients. Supports cardiothoracic surgeons in clinic / office setting. Assists as needed with patient education. Communicates across health care areas to ensure that systems integrate efficiently in order to best serve patients, staff, physicians, and community.

Schedule

• Full time 40 hours a week
• 8:00-4:30
• No Weekends or Holidays

General Requirements

• Coordinates with MercyOne OR surgery scheduling and cardiothoracic surgery team scheduler optimizing surgery schedule and timely care of cardiothoracic surgery patients,
• Responsible for educating patients and families regarding care plans outlined by cardiothoracic surgeons for patients
• Follows through with test results and plan of care with patient/family and other health care providers/services as directed by provider or established guidelines
• Acts as a liaison among the individual/family, community, provider and physician as well as other health care team members.
• Coordinates appropriate educationalmaterial/instructionsfor patients, families, medical staff and multidisciplinary personnel to accomplish patient care outcomes.
• Collaborates withpatients/families/populationsto coordinate community services and appropriate resources which meet the individual needs and goals.
• Is accountable for the implementation and communication of standards for patient care delivery appropriate to the specific patient population that meets professional and regulatory standards.
• Coordinates the health care team in development of clinical tools for optimal outcomes and reports findings.

Education

• Current Iowa registered nurse (RN) license.
• Bachelor’s degree in health-related field required or actively pursuing. BSN preferred.
• Demonstrates competency within a specialized area of clinical practice with 4 years experience in that specialty.
• Proof of completion of Mandatory Reporter – Child and Dependent Adult Abuse training within three (3) months of hire.
• Basic Life Support certified within six (6) weeks of hire. Acceptable credentialing bodies and certifications include American Heart Association Basic Life Support for Healthcare Providers.
• Demonstrates success in positions of leadership.
