Intermountain Heart & Vascular Wheat Ridge is seeking an RN Care Coordinator to support heart failure patients and participate in the diuresis clinic. You will perform comprehensive assessments, develop care plans with physicians, provide patient education, and coordinate services to improve outcomes.

In this full-time role, you will manage patient panels, collaborate with the care team, and engage with payers and community resources to promote cost-effective, patient-centered care.

Qualifications

• Comprehensive assessment of patients' health needs and psychosocial status.
• Ability to explain health information clearly to patients and families.
• Experience coordinating care across multiple disciplines is a plus.

Responsabilités

• Perform comprehensive assessment of patients' health needs, including health status, function, and support systems.
• Establish care plans in collaboration with physicians and patient care team.
• Provide health education and coordinate referrals to nutrition, social work, rehab, diabetes education, etc.

BLS certification

ACLS/PALS certification

BSN preferred/BSN pursuit
