Qualifications

• Three years general nursing experience.

Responsibilities

• Conducts new patient interviews and assesses patient function and educational needs.
• Monitors patient lab values, test results and progress.
• Provides patient/family education.
• Coordinate discharge needs for smooth transition from hospital to outside agencies.
• Acts as liaison between patient and other medical providers regarding progress.
