Job summary

Intermountain Health Heart & Vascular Wheat Ridge is seeking an RN Care Coordinator to support heart failure patients and a diuresis clinic. The role collaborates with physicians to coordinate care, educate patients and families, and monitor outcomes.

The position requires an active RN license, BLS, and applicable clinical certifications. It is a full-time, on-site role with a 40-hour work week and a competitive hourly wage.

Qualifications

• Current RN License in state of practice.
• BLS certification for healthcare providers.
• ACLS/PALS or equivalent as required by setting.
• Ability to communicate effectively both verbally and in writing.
• RNs with <12 months experience must obtain BSN within five years.

Responsibilities

• Perform comprehensive assessment of patients' health needs and determine potential needs.
• Establish care plans with the primary care physician and care team.
• Provide health education and coordinate care across services.
• Identify patients at risk for proactive intervention.
• Refer patients to nutrition, social work, rehabilitation, diabetes education and other resources.
• Coordinate care for complex cases and post-discharge follow-up.
• Maintain accurate documentation to support care team decisions.
