Job summary

WVUH West Virginia University Hospitals is seeking an HVI Care Coordinator to coordinate comprehensive patient care for Heart & Vascular Institute patients in Morgantown. You will participate in rounds, develop care plans, and coordinate discharge planning with the clinical team.

The role requires RN licensure in-state or through the eNLC, BLS within 30 days, and at least two years of nursing experience. Strong communication and teamwork are essential to support patient outcomes and education.

Qualifications

• Current RN license in the state where services will be provided or a multi-state RN license via eNLC.
• BLS certification must be obtained within 30 days of hire.

Responsibilities

• Manages comprehensive and individualized care for assigned patients and their families in the Heart & Vascular Institute.
• Assists team with daily rounds; conducts pre-procedure evaluations.
• Reviews and communicates diagnostic results per clinical guidelines and scope of practice.
• Coordinates discharge planning and follow-up services with clinic/hospital staff.
• Provides education and serves as a resource for clinical staff; participates in education programs.
