Job summary

Elliot Health System is seeking a dedicated professional to join its Cardiac Rehabilitation Program. You will develop and implement education, exercise plans, and prescriptions for adults participating in rehabilitation services guided by AACVPR standards.

Responsibilities include leading education on heart health, conducting intake sessions, assessing risk factors, and delivering a tailored exercise program. Timely documentation of patient progress is required, with per diem scheduling.

Qualifications

• Bachelor of Science in exercise science, exercise physiology, physical therapy or physical education.
• BLS, Required, or ability obtain immediately upon hire through Elliot
• Prior experience in clinical cardiac rehabilitation or equivalent, Required

Responsibilities

• Provides education on cardiac and vascular disease, risk factor management and healthy lifestyle modifications through group education classes and individual counselling sessions.
• Conducts interview/intake session with new patients for Outpatient Cardiac Rehabilitation, articulating benefits of the cardiac rehabilitation.
• Determines patient relative to risk factor management based upon the interview, pertinent laboratory, and medical history information.
• Conducts an exercise program which is appropriate for a cardiac and vascular population which will include, but not be limited to: warm-up, stretching, range of motion, cardiovascular conditioning, cool down activities, and resistance training; modifies exercise when patient’s physical or medical condition requires.
• Completes timely documentation of assessments, interventions, and evaluations of the Cardiovascular Health patient.
