Job summary

SolutionHealth is seeking a Per Diem Cardiac Rehabilitation Educator to join our Cardiovascular Health program in Manchester, NH. You will develop and implement education, exercise plans, and prescriptions for adult patients participating in the Elliot Health System Cardiac Rehabilitation Program following AACVPR standards.

You will educate patients on disease risk factors, lead group classes and one-on-one sessions, and assess risk factors to tailor individualized rehabilitation plans.

Qualifications

• BLS Certification required or ability to obtain immediately upon hire.
• Prior experience in clinical cardiac rehabilitation or equivalent required.
• Bachelor of Science in exercise science, exercise physiology, physical therapy or physical education required

Responsibilities

• Provides education on cardiac and vascular disease, risk factor management and healthy lifestyle modifications through group education classes and individual counselling sessions.
• Conducts interview/intake session with new patients for Outpatient Cardiac Rehabilitation, articulating benefits of the cardiac rehabilitation.
• Determines patient relative to risk factor management based upon the interview, pertinent laboratory, and medical history information.
• Conducts an exercise program which includes warm-up, stretching, range of motion, cardiovascular conditioning, cool down activities, and resistance training; modifies as needed.
• Completes timely documentation of assessments, interventions, and evaluations of the Cardiovascular Health patient.

About the Job:

Responsible for developing and implementing the education, exercise plan and prescription for adult-sized adolescent and adult participating in the Elliot Health System Cardiac Rehabilitation Program following the standards as set forth by the American Association of Cardiovascular and Pulmonary Rehab (AACVPR).

What You'll Do:
