Job summary

Remote Co. seeks an experienced Cardiac Rehab Exercise Physiologist to facilitate cardiac rehabilitation and risk-factor modification for patients. The role includes evaluating patients, recording progress, and delivering both in-person and virtual rehab sessions to support long-term lifestyle changes.

Responsibilities cover documentation, goal setting, collaboration with the care team, and patient education. Per diem schedule and outpatient availability may apply.

Qualifications

• Minimum degree in physical therapy or exercise science that qualifies practicing rehab.
• Experience with cardiac rehab population preferred.
• ACLS certification and ability to work with telemetry training beneficial.

Responsibilities

• Assess patients using approved evaluation tools and set realistic goals.
• Develop individualized treatment plans including exercise prescription and progression.
• Provide patient and family education on exercise, safety, and risk reduction.
• Deliver virtual cardiac rehab visits using Zoom when needed.
• Document patient progress and maintain accurate records in systems.
