Job summary

HealthPartners is seeking a Cardiac Rehab Exercise Physiologist to assess, educate, and restore maximum function for adults and seniors with heart, pulmonary, or vascular conditions at our hospital location.

The role emphasizes evidence-based exercise prescription, patient education, documentation, and collaboration with Rehab team to ensure safety, quality care, and optimal outcomes.

Qualifications

• BS or BA in Exercise Physiology required.
• Masters degree preferred.
• 1+ year experience in similar setting preferred.
• Age-related growth and development principles necessary to provide appropriate service and assure patient safety.

Responsibilities

• Provides evaluation, treatment planning, and treatment implementation to adult and geriatric clients; addresses development, cognitive and physical needs and cultural differences.
• Coordinates client care with other disciplines through good communication and intervention recommendations.
• Provides education, information, and training to clients, family, caregivers, and community to promote continuity of care and healthy lifestyle.
• Maintains documentation and records of treatment performance to assure quality client care and regulatory standards.
• Acts as a consultant to internal and external customers.
• Maintains education, knowledge, and training to provide up-to-date care and meet licensure requirements.
• Participates in quality improvement and peer review to assure effective services.
• Performs all other related duties as assigned.
