Job summary

Qualifications

Responsibilities

• Assess social determinants of health impacting care and engage appropriate resources.
• Develop cost-effective plans of care with the care team to achieve patient goals.
• Coordinate referrals and follow-up with PCP and other providers.
• Ensure compliance with CMSA Standards of Practice for Care Management.
