Job summary

Qualifications

Responsibilities

• Performs a comprehensive assessment on all assigned patients with complete documentation.
• Communicates changes in patient condition promptly to the care team.
• Uses nursing diagnoses and assessment data to set measurable goals in the care plan and adjusts as needed.
• Collaborates with patient, family, and health care team for care planning and discharge planning.
• Evaluates patient responses to interventions and updates the care plan accordingly.
• Includes discharge instructions, medication reconciliation, and patient portal education in the discharge process.
