Job summary

Responsibilities

• Coordinate placement of elective and emergent admissions and transfers.
• Ensure patients are admitted or transferred to the most appropriate bed.
• Communicate and implement Capacity Management plan for the day.
• Review patient conditions for appropriate placement with case management.
• Track bed availability and maintain logs/records.
• Work with Plant Ops and Environmental Services on Room of the Day.
• Conduct walking rounds to identify barriers to discharge.
• Monitor bed availability and pending discharges to meet ADT needs.
• Act as a resource for patient placement staff regarding unit acceptance criteria.
• Communicate with the Director for Capacity Management regarding needs.

What You'll Do

• Coordinate the placement of elective and emergent admissions and transfers from other facilities.
• Ensure patients are admitted or transferred to the most appropriate available bed.
• Communicate and implement the Capacity Management plan for the day.
• Review patient conditions for appropriate placement in conjunction with case management staff.
• Track bed availability and maintain appropriate logs/records.
• Work with Plant Ops and Environmental Services on daily Room of the Day (ROD).
• Conduct walking rounds on nursing units to identify barriers to discharge.
• Monitor bed availability and pending discharges to meet ADT needs.
• Act as a resource for patient placement staff regarding unit acceptance criteria.
• Communicate with the Director for Capacity Management regarding operational needs.
