Job summary

UChicago Medicine seeks a Supervisor for Cancer Community Health Workers to lead a CHW team across Ingalls Memorial Hospital and the UChicago Medicine Comprehensive Cancer Center in Hyde Park. You will supervise daily activities, evaluate CHW performance, and ensure timely attendance and program progress.

The role requires CHW leadership, coaching, data monitoring, and collaboration with program staff. A CHW background and strong communication skills are essential for success.

Qualifications

• High School Diploma or Equivalent; Bachelor’s Degree preferred.
• 5+ years of CHW experience; trained CHW.
• Valid driver’s license and own car.
• Familiarity with Chicago’s South Side communities.
• Formal CHW training completed.

Responsibilities

• Provide intensive supervision to the CHW team and evaluate CHWs’ progress.
• Conduct 1:1 meetings, coaching, and develop CHW training materials.
• Monitor data entry, track patients/community members, and report to Manager of Community Benefit.
• Assist in developing program models and curriculum for new CHW programs.
