Job summary

South Side Healthy Community Organization (SSHCO) is seeking a CHW Supervisor to provide leadership for a team delivering community health services across Chicago’s South Side. The role includes staff supervision, health education, and case management with occasional home visits and travel to multiple sites.

The ideal candidate has at least three years in public health, strong leadership and communication skills, and is committed to health equity.

Qualifications

• Flexible work schedule required.
• Travel throughout the Chicagoland area; valid driver's license and proof of insurance required.
• High school diploma or equivalent required; college degree preferred.
• At least three years in public health, social services or healthcare with a bachelor’s degree or equivalent.
• CHW certification preferred; up-to-date vaccinations per CDC guidelines.

Responsibilities

• Supervise and mentor CHW/CHOWs in educating and disseminating health messages.
• Guide CHW/CHOWs in connecting community members to health and social services and navigating case management.
• Lead CHW/CHOWs in recruitment, outreach, and ongoing care including home visits and phone consultations.
• Collaborate with project leaders to assess intervention effectiveness and manage caseloads.
• Participate in hiring, evaluations, and establishing professional boundaries with staff.
