Qualifications

• Associate degree required; Bachelor's preferred.
• 2+ years in Oncology care coordination/related experience preferred.
• Experience coordinating care across inpatient to outpatient settings.

Responsibilities

• Coordinate clinic scheduling and tests to support diagnosis and treatment.
• Facilitate communication between patients, families, and healthcare providers.
• Reinforce the clinical care plan and ensure continuity of care.
