Job summary

Aurora Health Care in Wisconsin is seeking a Cancer Care Coordinator to ensure continuity of care for patients through the cancer journey, from diagnosis to treatment and follow-up. You will build relationships with patients and their primary care teams to navigate care pathways and personalize support.

You will perform comprehensive nursing assessments, collaborate with multidisciplinary teams, educate patients and families, and connect them with resources to improve outcomes and satisfaction

Qualifications

• Registered Nurse license required
• BLS certification required
• 5 years of clinical nursing experience preferred
• Experience in cancer care or care management a plus

Responsibilities

• Provides continuity of care across care settings according to clinical pathways.
• Completes nursing assessments and develops plans of care with other disciplines.
• Guides patients with suspected cancer through the diagnostic phase and follow-up.
• Communicates with physicians, hospital staff and community resources.
• Refers patients to financial counseling, counseling services, trials, and support groups.
• Collaborates with leadership to develop and maintain quality programs.
• Educates patients and families on prevention and early detection of cancers.
• Promotes community education and outreach related to cancer services.
• Utilizes data to implement best practices and improve outcomes.
