Job summary

Qualifications

• Bachelor of Science in Nursing required
• Current, unrestricted California RN license required
• Professional Certification in area of specialty required within one year of hire
• A minimum of 3 years of nursing experience in an acute care setting (transplant, cancer)
• Experience with program development and implementation of navigator programs preferred

Responsibilities

• Coordinate care for patients throughout the continuum of treatment, serving as a consistent contact and advocate
• Review, evaluate and revise the plan of care for appropriateness on an ongoing basis
• Develop guidelines, protocols, pathways, and other performance improvement activities related to patient care
• Collaborate with the interdisciplinary team and the Patient Advisory Council to educate and improve programs for patients and the community
• Support program growth and ensure excellent patient- and family-centered care under the Executive Director
