The Nurse Navigator is assigned to a population of patients and provides individualized and comprehensive care to reduce barriers and ensure prompt access to care. Utilizing the nursing process, the Nurse Navigator performs clinical assessments, educates on disease processes, and manages symptoms to support patients through their care continuum. The Nurse Navigator collaborates and communicates effectively with the multidisciplinary team to coordinate patient care, ensure continuity of care, and provide an exceptional patient experience. The Nurse Navigator initiates referrals to appropriate inter‑disciplinary teams for physical, psycho‑social, and spiritual support through all stages of the continuum of care. The nurse navigator also collaborates with the social worker and supports patient navigation and social supportive items. The nurse navigator will support quality care by participating in and supporting quality improvement activities. The role upholds and applies standards established by the American College of Surgeons to ensure continued accreditation as a Community Cancer Center.
Please be advised that this location requires quarterly American Heart Association (AHA) Resuscitation Quality Improvement (RQI) competency certification post‑hire. This ongoing competency is required through internal resources; outside RQI competency resources are not acceptable for CommonSpirit Health compliance requirements.
