Job summary

Whitman-Walker Health is seeking a Cancer Patient Navigator to serve as the primary contact for participants in its Cancer Navigation Program. The role delivers culturally responsive, trauma-informed navigation across the cancer care continuum, coordinating insurance, appointments, and supportive services to improve access and engagement in care.

The Navigator works with clinical teams, oncology partners and community organizations to address social determinants of health and to reduce barriers

Qualifications

• Excellent communication in person, writing and by phone with diverse audiences.
• Experience in fast-paced, complex environments with competing priorities.
• Knowledge of social determinants of health and oncology navigation practices.

Responsibilities

• Serve as primary contact for Cancer Navigation Program participants.
• Conduct comprehensive needs assessments to identify barriers to care.
• Provide patient education on cancer prevention, screening, treatment and follow-up.
• Assist with insurance enrollment, benefits navigation, and financial resources.
• Coordinate referrals and appointments; facilitate communication with oncology providers.
• Connect patients to transportation, childcare, housing, food access, and other supportive services.
• Document interactions in the EHR and tracking systems.
• Collaborate with multidisciplinary teams and participate in outreach and education.
