Job summary

Dana-Farber Cancer Institute seeks an Oncology Nurse Navigator to guide patients, families and caregivers through the cancer journey. Using nursing knowledge they educate patients, coordinate services, and help overcome barriers to timely, quality care.

In this role you will perform clinical nursing tasks, conduct telephone triage, monitor symptoms, manage patients on oral chemotherapy, and update charts while partnering with the health care team to align goals of care and enhance patient

Qualifications

• BSN preferred for this nursing navigator role.
• Minimum 1 year of RN experience required.
• Certification in oncology or related fields is a plus.

Responsibilities

• Assumes nursing care responsibilities for a defined group of patients within the program.
• Offers education and resources to facilitate informed decision making and timely access to care.
• Coordinates care, documents updates, and serves as a liaison among patients, families and providers.
• Provides telephone triage and symptom management.
• Supports transition of patients through treatment, survivorship or end-of-life planning.
