Responsibilities

• Enroll patients in IBCCP and track budgets and caseloads in Cornerstone.
• Provide case management for cancer patients and coordinate care with RN oversight.
• Navigate care for high‑risk or newly diagnosed cancer patients, especially breast cancer.
• Assist patients in understanding diagnosis, treatment options, and resources.
• Coordinate multiple support groups and events for cancer survivors.
