Job summary

Erie Family Health Centers is seeking a proactive IBCCP Patient Navigator to enroll patients, manage cases, and coordinate cancer support for breast, cervical, and colorectal cases. The role requires bilingual English/Spanish communication and experience in healthcare settings.

You will work with RN oversight, assist patients with diagnosis and treatment options, and link to community resources while contributing to our mission of providing high-quality care regardless of insurance or income.

Qualifications

• High School diploma or equivalent required.
• AA/AS in Community Health or Health Education, LPN, MA, or equivalent preferred.

Responsibilities

• Enroll patients in IBCCP and track caseload.
• Provide case management for cancer patients with RN oversight.
• Coordinate patient navigation for high-risk and newly diagnosed cancers (breast, colorectal, cervical).
• Assist patients in understanding diagnosis and treatment options, and connect to resources.
