Job summary

Qualifications

• Associate degree or equivalent experience in social work or related field.
• At least 3 years in case management or direct service in social services.
• Ability to build rapport with individuals with diverse disabilities.
• Experience with EHR systems, preferably MiCil.
• Strong organization and time management with large caseload.
• Valid CA driver’s license and reliable transportation; travel across service area.
• Ability to lift up to 20 lbs and stand/walk for extended periods.

Responsibilities

• Develop and cultivate partnerships with landlords, housing complexes, hospitals, and other facilities to facilitate transitions.
• Collaborate with landlords and discharge staff to ensure services are in place prior to transition.
• Conduct comprehensive care plan assessments and eligibility screenings.
• Provide direct support and advocacy to individuals throughout the transition process.
• Coordinate and deliver essential goods and items to program participants.
• Maintain accurate case files and documentation; enter data into MIS.
• Participate in regular case staffing meetings.
