Qualifications

• Must be 18 years of age or older.
• Previous supervisory experience in case management.
• Minimum two years providing case management to individuals with complex needs.

Responsibilities

• Supervise and mentor care managers, providing feedback.
• Develop processes for client and staff care.
• Conduct in-depth needs assessments and develop care plans.
• Engage in community outreach for client support.
