Job summary

ELEVATE seeks a Substance Use Counselor in Iowa with a CADC or tCADC license. The role centers on new patient assessments and intake documentation, including biopsychosocial and initial ASAM, and crafting the initial treatment plan for those entering Intensive Outpatient or Partial Hospitalization.

Responsibilities include facilitating groups (rotating Saturdays), running urinalyses at admission, and providing individual and group counseling while coordinating with the care team to meet

Qualifications

• Must have CADC or tCADC license in Iowa.
• Bachelor's degree in human services or similar preferred.
• Experience with substance use disorders/co-occurring mental health issues required.
• Strong documentation and intake assessment skills.

Responsibilities

• Assess applicants to determine level of care.
• Admit client and complete required documentation.
• Perform intake assessments using ASAM, Columbia, Suicide Risk Assessment, Biopsychosocial assessment, etc.
• Add client to the IBHRS database.
• Coordinate new client admissions using the Client Orientation Handbook.
• Collaborate with peers and staff to provide highest level of care.
• Conduct client urinalysis tests at admission.
• Develop initial treatment plans for IO/ PHP levels.
• Provide individual and group counseling with measurable goals.
• Coordinate with other professionals and community services.

Education
