Job summary

The City and County of San Francisco seeks a Social Work Specialist (CAAP) to provide case management to eligible CAAP recipients. You will conduct assessments, help clients apply for disability benefits, and connect them with community resources to support physical and mental well-being.

You will collaborate with physicians, SSA representatives, and other stakeholders, educate clients on SSA/CAPI processes, and stay updated on related policies.

Qualifications

• Bachelor's degree from an accredited college or university.
• Two years of social casework or employment services experience with a public or private agency/organization.
• Substitution for Education: additional experience may substitute for the degree (up to two years).
• Substitution for Experience: Master's degree in Social Work or Counseling may substitute for one year of experience.

Responsibilities

• Provide case management services to CAAP clients and assess their situation through interviews and biopsychosocial assessments.
• Summarize clients' functional limitations and make referrals to community services; collaborate with physicians, SSA representatives, and other stakeholders.
• Educate clients and help navigate the SSA and CAPI application processes for disability benefits.
• Serve as liaison with SSA, State Department of Social Security, and other governmental organizations; maintain contact with disability examiners.
