Job summary

FirstLight Home Care of West St. Paul is seeking a relationship-driven outreach professional to grow our home care services through community partnerships.

This role focuses on building relationships with hospitals, senior communities, hospice and local organizations to generate referrals and support families in need. You’ll visit partners, follow up on referrals, attend networking events, and help track activity and results to demonstrate impact.

Qualifications

• Strong relationship-building and communication skills.
• Experience in outreach or healthcare partnerships is preferred.
• Ability to work in a fast-paced community environment.
• Comfortable visiting partners in person and on-site.

We are looking for a relationship-driven professional to grow our home care services through community outreach and healthcare partnerships.

This role focuses on building relationships with hospitals, senior communities, hospice, and local organizations to generate referrals and support families in need.

What You’ll Do

• Visit healthcare and community partners (10–15 visits/week)
• Build relationships with social workers, nurses, and care teams
• Generate and follow up on referrals
• Attend networking events and host Lunch & Learns
• Track activity and results weekly

Compensation

• $250 per new client start
• $1 per service hour generated (bonus)
• Quarterly bonus: $1,000–$3,000
