Job summary

Wesley Medical Center in Wichita, KS is seeking a Social Worker (BSW) to promote patient‑centered care by evaluating psychosocial needs and coordinating community resources for identified patient populations. The BSW serves as a liaison between patients and post‑acute providers and develops timely discharge plans across Med/Surg units.

Qualifications

• Bachelor’s degree required.
• LBSW or LMSW/RCSW-I with eligibility required.
• BLS certification must be obtained within 30 days of start date.

Responsibilities

• Collaborate with RN case managers and clinicians to identify and address patient needs.
• Identify barriers to discharge including financial, emotional, psychosocial, medical, and spiritual factors.
• Perform assessments and develop discharge plans for high‑risk patients with discharge planning needs.
• Obtain input from RN case managers to ensure discharge plans align with the patient care plan.
• Reassess and modify discharge plans as needed in coordination with the clinical team.
• Participate in interdisciplinary care planning and proactive discharge planning.
• Educate patients and caregivers on community resources and post‑acute care criteria, benefits, and coverage.
