Job summary

CHRISTUS Health is seeking a Social Worker to interview patients and relatives to obtain social history and assist with environmental barriers to care. You will serve as a liaison between medical and nursing staff, patients, relatives and outside agencies, and help in discharge planning and community resource referrals.

Qualifications

• Bachelor's degree required.
• LBSW licensure in the state of employment required.

Responsibilities

• Meet patient needs for discharge planning through the continuum of care.
• Direct access to community and adjunct resources to foster quality of life.
• Interview patients and family members to obtain psychosocial assessments.
• Assist patients and families with adjustments to illness and disabilities and resolve financial and environmental difficulties affecting care.
• Provide crisis intervention and individual/family therapy when appropriate.
• Coordinate discharge arrangements starting at patient admission.
• Participate in interdisciplinary rounds to create discharge plans.
• Assess psychosocial needs of families that may affect care management.
• Refer patients to community resources to enhance quality of life.
• Refer to counseling services and other recognized psychosocial therapies as needed.
• Initiate referrals and appear in court if required.
• Provide care aligned with age-specific needs of patients.
• Refer to Spiritual Care and assist in crisis situations.
• Identify high-risk patients with maladaptive behaviors.
