HCA Houston Healthcare Clear Lake seeks a Social Worker (BSW) to promote patient-centered care by evaluating psychosocial needs and coordinating discharge planning on Med/Surg units. You will work with the care team to identify barriers to discharge and arrange post‑acute interventions.

You will educate patients and families on community resources, facilitate meetings, and support informed decisions while honoring patient preferences and values.

Qualifications

• Licensed social worker credential (LBSW) required.
• Bachelor's degree in social work or related field.

Responsibilities

• Collaborate with the care team to identify patient needs and resources.
• Develop and coordinate discharge plans for high‑risk patients.
• Address barriers to discharge (financial, emotional, psychosocial, medical, spiritual).
• Educate patients and families on post‑acute care options and coverage.
• Facilitate meetings with patients/families and care teams to support decision making.
