Job summary

Qualifications

Master's Degree in Social Work from accredited institution.
Strong interview, assessment, organizational and problem-solving skills.
Excellent interpersonal communication and negotiation skills.
Ability to work with people of diverse backgrounds.
Licensure as LCSW/LMSW in state of employment is required or preferred.

Responsibilities

Conduct psychosocial assessments with age-specific focus.
Assist with discharge planning and linkage to post-acute resources.
Advocate for patient and family empowerment and autonomy in care decisions.
Collaborate with RNs, physicians, and the interdisciplinary team to coordinate care.
Identify and mobilize community resources to improve quality of life.
