Job summary

Trinity Health Grand Rapids Hospital is seeking a Social Worker (BSW) to support discharge planning and patient care across med/surg, ICU, mother/baby, psychiatric medical, and ED units. The role involves psychosocial assessments and coordinating resources.

The Social Worker will collaborate with discharge planners and nurses, educate patients and families, and ensure timely follow-up and referrals in a patient-centered, confidential environment.

Qualifications

• Bachelor of Social Work (BSW) degree required.
• Current Michigan licensure as a licensed Social Worker.
• One year of social work experience in a healthcare setting.

Responsibilities

• Complete psychosocial assessments and community service planning.
• Support discharge planners and RN Case Managers.
• Connect patients with community resources to address social needs.

Employment Type:Full time Shift:Day Shift Description:This is a BSW position to help support the Clinical Social Workers and Nurse Case Managers at Trinity Health Grand Rapids Hospital. The units covered are med/surg, ICU, mother/baby, Psychiatric Medical, and Emergency Department.

This role provides and facilitates patient care within the framework of established policies, procedures, standards, and guidelines for patients at Trinity Health Grand Rapids Hospital.

What the Social Worker (BSW) will do:

• Completes psychosocial assessment and community service planning.
• Provide support to the hospital discharge planners (RN Case Managers and Master's prepared Social Workers)
• Connect patients with appropriate community resources to address their Social Needs
• Facilitate referrals for discharge planning purposes
• Provide appropriate education for patients and families.
• Arranges transportation as needed for hospital discharges
• Coordinate hospital follow up appointments
• Participate in an interdisciplinary team to ensure successful and efficient discharge planning for hospitalized patients
