Required are a BSN and active Illinois RN license, plus Basic Life Support. The nurse will support ethical practice, patient education, and quality metrics while delivering high-quality care throughout the patient journey.

Qualifications

• Bachelor of Science in Nursing (BSN) is required.
• Current State of Illinois Registered Professional Nurse is required.
• Basic Life Support certification is required.

Responsibilities

• Performs a comprehensive assessment on all assigned patients with documented evidence.
• Communicates changes in patient's condition promptly.
• Uses nursing diagnoses and assessment data to set measurable goals in the plan of care and updates as needed.
• Identifies and prioritizes patient needs and/or nursing diagnoses and documents.
• Collaborates with patient, family, and health care team to plan care and coordinate discharge planning.
• Evaluates patient responses to interventions and adjusts the plan of care accordingly.
• Discharges planning and appropriate referrals, including palliative care, when indicated.
• Ensures discharge instructions, medication reconciliation, and portal usage information are provided.
