Job summary

Comunilife is seeking experienced Case Managers for the El Rio Residence Program in the Bronx. The role focuses on providing supportive, person-centered services to program participants in a housing setting, using Harm Reduction to support recovery and opportunities.

Responsibilities include conducting assessments, developing service plans, coordinating access to medical care, and assisting with employment and education referrals.

Qualifications

• Bachelors Degree required.
• minimum two years field work experience.
• direct mental illness work experience.

Responsibilities

• Provide comprehensive case management services to clients in housing settings.
• Develop service plans with long and short-term goals based on assessments.
• Conduct home visits and office visits weekly as per service plans.
• Assist clients with accessing primary medical care and adherence to medications.
• Provide counseling and orientation regarding tenant rights and responsibilities.
• Coordinate referrals to medical, home care, mental health and substance abuse services.
• Collaborate with vocational counselors and entitlement specialists for education and employment programs.
• Maintain documentation of all client contacts and progress.
