New York Health (NYH) in Bohemia, NY seeks a Breast Nurse Navigator (RN) to guide patients through biopsy prep, results and follow-up coordination. You will educate patients by phone, review pathology, and communicate with radiologists while navigating referrals in the EMR.

The role requires NY RN licensure, strong assessment and communication skills, and some bilingual English/Spanish capability is a plus. Competitive pay and benefits offered.

Formación

• Graduate from an accredited School of Nursing and hold a New York RN license.
• Nursing experience preferred and highly regarded.
• Excellent assessment, communication and organizational skills.
• Good computer knowledge; bilingual EN/ES a plus.

Responsabilidades

• Educate patients on biopsy prep, post-procedure expectations and follow-up via phone.
• Review, track and document pathology results and notify radiologist of findings.
• Notify patients of benign results and explain recommendations clearly.
• Assist patients with follow-up appointments and referrals in the EMR.
• Outreach to patients needing additional imaging or follow-up after mammography.
• Support patients due/overdue for annual mammography and maintain EMR documentation.
