Job summary

New York Health in Bohemia, NY seeks a Breast Nurse Navigator (RN) to educate patients on biopsy prep, post‑procedure steps, and results communication. You will review pathology, coordinate follow‑ups, and document in the EMR.

This role requires NY RN licensure and strong communication skills. Responsibilities include guiding patients through referrals, staying on top of imaging follow‑ups, and ensuring timely, accurate documentation.

Qualifications

• Graduate from an accredited School of Nursing with NY license
• Nursing experience preferred
• Excellent assessment, communication and organizational skills
• Bi‑lingual English/Spanish a plus

Responsibilities

• Provide education regarding biopsy preparation, post‑procedure expectations, and follow‑up instructions via phone call
• Review, track, and document pathology results, ensuring findings are communicated to the radiologist for review
• Notify patients of all benign pathology results and ensure understanding of recommendations
• Assist patients in navigating follow‑appointments and referrals and ensure orders are placed correctly in the EMR
• Conduct outreach to patients requiring additional diagnostic imaging or follow‑up after mammography screenings
• Communicate with and assist patients due or overdue for annual mammography screenings
• Maintain accurate and timely documentation in the EMR
