Job summary

New York Health is seeking a Breast Nurse Navigator (RN) at Bohemia, NY. You will educate patients, review pathology results, and guide them through follow‑ups and referrals.

The role includes outreach after mammography and ensuring timely EMR documentation, with a focus on compassionate, evidence‑based patient care. Requirements include NY RN license and nursing degree; bilingual English/Spanish is a plus.

Qualifications

• Graduate from an accredited School of Nursing and hold an unencumbered New York RN license.
• Strong assessment, communication and organizational skills.
• Proficiency with EMR and clinical software.
• Bilingual English/Spanish is a plus.

Responsibilities

• Educate patients on biopsy prep, post‑procedure expectations and follow‑up instructions by phone.
• Review, track and document pathology results and communicate findings to radiologists.
• Notify patients of benign pathology results and explain recommendations.
• Assist patients in navigating follow‑up appointments and referrals in the EMR.
• Outreach to patients needing additional imaging or follow‑up after mammography.
• Communicate with and assist patients due for annual mammography screenings.
• Maintain accurate and timely documentation in the EMR.
