Job summary

Salinas Valley Health Clinics seeks an experienced Breast Coordinator RN to oversee coordination of care across the breast health continuum. You will liaise with radiology, surgical teams, and oncology to ensure timely access to diagnostics, biopsy and treatment, while educating patients and families on care plans and survivorship resources.

The ideal candidate has a BSN and at least two years of nursing experience, with current RN and BLS certification.

Qualifications

• Graduate of an accredited school of nursing.
• Minimum of two years clinical nursing experience required.
• Current RN license; BLS certification required.
• BSN preferred.

Responsibilities

• Coordinate patient care across the breast health continuum, from screening to follow-up.
• Serve as the clinical liaison among patients, radiologists, PCPs, surgeons, oncologists, and ancillary services.
• Facilitate timely scheduling and follow-up of abnormal imaging findings and procedures.
• Monitor care pathways to reduce delays in diagnosis and treatment.
• Provide patient education on imaging, procedures, survivorship, and resources.
• Ensure access to resources: financial counseling, transportation, language services, and psychosocial support.
