Job summary

Ravenswood Family Health Network is seeking a Breast and Cervical Cancer Screening Program Assistant to manage patient tracking for screening services at multiple clinics. The role involves outreach, data entry in the EHR, and coordination with the BCCTP and Medi-Cal programs.

The ideal candidate will have a High School diploma or GED, one year of health care outreach experience, and strong communication skills.

Qualifications

• High School diploma or GED required.
• One year of health care or social services outreach experience preferred.
• Excellent interpersonal, oral, and written communication skills.
• Bilingual Spanish preferred.

Responsibilities

• Track and follow up with patients who are overdue for breast and cervical cancer screening.
• Maintain accurate records and document interactions in the EHR.
• Enroll patients in programs and coordinate referrals to Medi-Cal and BCCTP as needed.
• Perform outreach calls and schedule imaging or follow-up appointments.
• Liaise between patients and program staff to address barriers to care and treatment.
