On-Site Community Health Navigator — Cancer Screening

Baylor College of Medicine seeks a Community Health Worker to support the CPRIT project, helping patients access timely cancer screening through education and resource navigation. This on-site role collaborates with the Project Coordinator and Director to follow established protocols.

The position requires a High School diploma or GED and at least two years of relevant health/social services experience. English and Spanish fluency are preferred to assist diverse patients.

Qualifikationen

• Experience using EMR systems.
• Strong communication, attention to detail, and organization.

Aufgaben

• Assist patients for cancer screening and intake per policy.
• Schedule breast/colorectal screening appointments and track calls.
• Conduct education and outreach per protocol.
• Link patients with resources and referrals to outside agencies.
• Maintain confidentiality of patient information (PHI).
• Communicate with patients in a compassionate, non-judgmental manner.
• Attend weekly project meetings and assist with clerical/administrative duties.
