CHRISTUS Health in Beaumont, TX is seeking a Breast Care Patient Navigator to support timely patient care and follow-up. This role acts as the patient advocate within the breast care team, helping patients navigate from suspicious findings to follow-up testing while addressing barriers to access and coordinating care.

You will work with an interdisciplinary team, arrange appointments for consultations and services, and help eligible patients access grants.

Formación

• Graduate of an accredited program in Radiologic Technology or equivalent.
• 1 year or more of breast imaging experience.
• Basic computer experience.
• Mammography (M) by ARRT is required.
• State licensure required: TX MRT, LA LRT, NM RRT.
• Breast Navigator Certification Program (BNCP) preferred.

Responsabilidades

• Helps patients navigate timely and quality care from suspicious findings to follow-up testing, addressing barriers to care.
• Facilitates appointments for consults and support services within established standards.
• Functions as a member of an interdisciplinary team to assess, secure, and coordinate appropriate services.
• Helps qualified patients secure services from state or national grants.
• Works with the Breast Care Team to collect data, track outcomes, and support strategic work plans.
• Participates in educational outreach to the community and key customers to maintain positive working relationships.
• Performs all other duties as assigned.
