Job summary

CHRISTUS Health in Beaumont, TX is seeking a Breast Care Patient Navigator to support timely patient care and follow-up. This role acts as the patient advocate within the breast care team, helping patients navigate from suspicious findings to follow-up testing while addressing barriers to access and coordinating care.

You will work with an interdisciplinary team, arrange appointments for consultations and services, and help eligible patients access grants.

Qualifications

• Graduate of an accredited program in Radiologic Technology or equivalent.
• 1 year or more of breast imaging experience.
• Basic computer experience.
• Mammography (M) by ARRT is required.
• State licensure required: TX MRT, LA LRT, NM RRT.
• Breast Navigator Certification Program (BNCP) preferred.

• Participates in educational outreach to the community and key customers to maintain positive working relationships.
• Performs all other duties as assigned.

Summary

Breast Care Patient Navigator serves as an essential part of the breast care team by supporting timely patient care and follow-up. This job is an advocate for the patient in their dealings with the healthcare community, helping patients navigate their way to timely and quality care and assisting the breast care team in patient care initiatives and programs.

Responsibilities

• Helps patients navigate their way to timely and quality care from suspicious findings to follow-up testing, while addressing tangible barriers to patient care.
• Facilitates appointments for consults and support services within established standards.
• Functions as a member of an interdisciplinary team to assess, secure, and coordinate appropriate services.
• Helps qualified patients secure services from any available grants from state and or national programs.
• Works with the Breast Care Team and others to collect data, track outcomes, and support strategic work plans.
• Participates in educational outreach efforts to the community and to key customers (physicians, office staff, diagnostic staff, etc.) to establish and maintain positive working relationships.
• Performs all other duties as assigned.

Job Requirements
